What is the 2021 Race-Free CKD-EPI Equation?

The 2021 CKD-EPI Creatinine equation estimates the glomerular filtration rate (eGFR) using serum creatinine, age, and sex. Unlike earlier formulas, it removes the race-based correction factor that historically adjusted results for Black patients. Eliminating that modifier promotes more equitable kidney care by ensuring every patient is assessed with the same standard. When a serum Cystatin C value is also available, the combined Creatinine–Cystatin C equation can provide a more precise estimate, since Cystatin C is less influenced by muscle mass and diet.

How is Pediatric eGFR Calculated?

For children, kidney function is estimated with the Modified Bedside Schwartz equation. It relies on the child's height and serum creatinine, multiplying height in centimeters by a constant (0.413) and dividing by creatinine in mg/dL. Because growing children have different body composition than adults, this height-based approach offers a practical, validated way to estimate creatinine clearance and screen for impaired kidney function in pediatric patients.
